Agrace Supportive Care

When you have a life-altering illness, you have a lot on your mind. Agrace Supportive Care can help you explore ways to feel more comfortable and less stressed, despite your illness. This specialized whole- person care (also called “palliative care”) comes to you at home. A nurse practitioner takes time to hear about your symptoms, your worries, hopes, goals and treatment options. We work with your other health care providers to help you live well.

Agrace Age at Home

With the right kind of help, today’s seniors can live where they want to be—in their own homes. Agrace Age at Home provides non-medical personal care that comes to your home. We can help with housekeeping, showering, dressing, meal prep, medication reminders, companionship and more. You choose the services you want, so you or a loved one can stay safer, happier and more independent at home.

Agrace Adult Day Center

Do you care for an older adult who cannot— or chooses not to—stay alone all day? The Agrace Adult Day Center in Madison gives seniors the reassurance and comfort of having others with them throughout the day. It’s especially helpful for people who have dementia or trouble with their memory. This state-certified day center gives a welcome break to family caregivers, too.

Agrace Hospice Care

Everyone hopes for comfort and dignity at the end of their life. Agrace Hospice Care is a blend of compassionate medical, emotional and spiritual care that comes to you and your family wherever you call home. Your care team visits regularly, and support is just a phone call away every hour of every day. Agrace also offers inpatient hospice and memory care options— and has served thousands of patients and their families across southern Wisconsin since 1978.

Agrace Hospice Memory Care Suites

When your loved one has advanced dementia, keeping them at home may not be manageable—even with in-home hospice care.
The Agrace Hospice Memory Care Suites in Madison are for people who need end-of-life care and extra support for their memory loss. Care is provided 24/7 by nurses who have specialized memory-care training. Residents pay a room-and-board fee to live in this secure, homelike space that’s specially designed to reduce their agitation, discomfort and need for hospitalization.

Agrace Grief Support

When someone you love dies, you may not know what to feel or how to keep living your day-to-day life. That’s OK, because Agrace’s grief support specialists understand—and know how to help. Agrace offers professionally led grief support groups for adults and children, one-to-one grief support, and grief support for schools and workplaces where people are coping with a death. Many Agrace support groups meet at the freestanding Agrace Grief Support Center in Fitchburg, while others take place in communities across southern Wisconsin—or even virtually with people attending them from home.
